Please use this email template for sending out relevant forms that will need to be filled out at various stages of the DMIN thesis presentation by the thesis committee.

I've attached the relevant forms that will need to be filled out at various stages of the DMIN thesis presentation.
It is recommended to use Google Docs between the DMIN examining committee for easier communication. Please make a copy of the Google docs as the links below are the original. I've listed the forms below in chronological order.
1. Thesis Project completion form - This form must be completed by the student. This will trigger an online reference form that the thesis supervisor will also complete.
2. Doctoral Project External Reader Appointment - The external reader must fill out this form via the link. If they have any issues or questions, they may email sto@carey-edu.ca or registrar@carey-edu.ca.
3. Initial (Pre-Defense) Assessment Form 2023 - This form is filled out by the thesis supervisor, second reader, and external reader before the thesis presentation/oral examination.
4. Committee Assessment of the Thesis Project and Oral Defense Form 2023 - This is the form that will be signed, completed and submitted immediately after the oral presentation.
5. DMin Thesis Approval Form 2023 - This form is to be signed, completed, and submitted after the thesis supervisor has received the thesis in it's correct format and with any major or minor revisions as recommended by the examining committee. When the thesis supervisor returns the completed form to me, please include the final grade. See the proposed grading scale documentthat will be approved at Senate in January.
6. Honorarium Form - Once the oral presentation is completed and the external reader has appended their signature to the DMIN Thesis Approval Form, they will need to complete this form so that our Accounts team can move forward with the payment.

7.Submitting Final Copies of the Thesis - the student must follow the formatting and submission instructions in this Fresh Desk article as the final step before graduating from the DMin program (including submitting the graduation application form). The student may only submit final copies to the Registrar's Office once the DMIN Thesis Approval Form has been completed by all signatories as the student will need to include the signature page in the digital copy of the thesis.
All Populi forms will be processed by the Registrar's Office. Any PDF Assessment and Approval forms must be submitted to Sarah (by email sto@carey-edu.ca) for processing.
